Application Instructions: Each section of this application must be completed in order to submit the application. Please review each section for accuracy before submitting your application. Upon completion of the application, your credit card will be charged and you will receive a confirmation via e-mail.
Please note that upon submission, your application will still be subject to review. Please allow a minimum of 3-4 weeks for your application to be reviewed. If approved, you will receive an e-mail notifying you of your approval to take the exam.
